Why CoCM with Profound

  • Whole-person behavioral health integrated into primary care (PCP, BHCM, psychiatric consultant).
  • Faster access and follow‑through via warm handoff + AI‑assisted workflows.

Pricing: fixed-fee $10 per Active Patient Month (APM); Net 15.
APM definition: a member with ≥1 qualifying CoCM interaction in the calendar month.
Audit-Ready Credit (ARC): if a denial is solely due to our documentation/coding defect, we credit that member-month on the next invoice.
No rev-share: fees are not tied to collections, payer mix, or minutes attained.

  • Capture and roll up billable minutes aligned to payer guidance.
  • Deliver audit-ready monthly artifacts to support your billing workflows.

What You Do

  • Identify and refer eligible patients during PCP visits (or via panel review).
  • Complete initiating visits and document consent; advise on cost‑sharing per your payer mix.
  • “You Bill” CoCM using the artifacts we provide and your existing RCM processes.

Timelines

  • Week 1: Kickoff, roster, contracts (BAA + services), clinic champion identified.
  • Week 2: SSO + integration smoke test, tablet setup for warm handoff, credentials exchanged.
  • Week 3: Dry run handoffs; confirm measurement cadence and escalation paths.
  • Week 4: Go-live and first-two-weeks SLA monitoring.

Billing Snapshot

  • Codes: 99492 (initial), 99493 (subsequent), 99494 (+30m add‑on), G0512 (FQHC/RHC), G2214 (first 30m option per payer/MAC policy).
  • Outputs: CSV minutes + summary; delivery by the 5th business day following month‑end.
  • Model: You Bill (your organization remains biller of record for CoCM).

  • HIPAA/BAA in place, encryption in transit and at rest, role‑based access with RLS, comprehensive audit logging.
